Welcome to our survey on Lyme in TX. The form provided is to be filled out by those who were infected by tick-borne illness in TX. It is solely intended to improve reporting, surveillance, mapping, and identifying gaps in access to care. Participation is confidential and asks for your email address, please share this within your groups, friends, and family where appropriate. Your address will not be sold, or used outside of Lyme advocacy in TX.
